Job summary

North Cumbria Integrated Care NHS Foundation Trust is seeking a Community Hospital Discharge Navigator to join the ToCH team. Based at Brampton with coverage across six sites, the role is full-time and fixed-term for maternity cover.

Travel to community hospitals is required to support discharge planning and patient flow, ensuring safe transitions from hospital to home or community settings. The post supports a multi-disciplinary team and aims to reduce length of stay through effective discharge

Qualifications

• Evidence of GCSE English and Maths or equivalent.
• NVQ 4 in Care or NVQ in Admin or equivalent experience.
• Knowledge of hospital discharge processes.
• Experience in hospital setting.
• Good IT skills and data recording.

Responsibilities

• Support the Transfer of Care Hub (ToCH) discharge team across pathways 1-3.
• Facilitate integrated discharge planning to ensure safe transfer.
• Assist patient flow and contribute to reducing length of stay.
• Liaise with patients, carers and multi-disciplinary teams to support discharge.
