Job summary

Westmorland and Furness Council is seeking a Trusted Assessor based at Allerdale House, Workington, to support safe hospital discharge and coordinate access to health and social care services.

Working with Case Managers and Discharge Navigators, the post focuses on timely transitions to community care, ensuring smooth pathways and appropriate support for adults after discharge.

Job description

Salary: £39,481 to £40,444 per annum

Hours: 36 hours per week

Location: Allerdale House, Workington

Contract: Temporary (6 months)

About this opportunity

As a Trusted Assessor within a hospital-based service, you will work collaboratively with Case Managers, Discharge Navigators, and multidisciplinary partners across all Discharge to Assess pathways. The role focuses on supporting and facilitating safe, timely, and effective discharges from hospital into a range of health and social care services, including both community-based and building-based provision.

Working as part of an integrated team, you will triage and coordinate access to the most appropriate services and support following discharge. You will help ensure individuals achieve a seamless transition from hospital to community care.

This role requires a DBS check. The level of check for this role is:Enhanced DBS check with a Adults Barred List check.
