Hampshire and Isle of Wight Healthcare NHS Foundation Trust seeks an Extended Clinical Practitioner to deliver hospital-level care in patients' homes as part of the Hospital at Home service. Qualifications

• Appropriate professional qualification enabling UK registration
• Current UK professional registration (NMC/HCPC)
• Degree-level award
• Independent non-medical prescribing or level 7 pharmacology/qualification
• Level 7 in Clinical History Taking & Physical Examination
• Level 7 in Diagnostics and Decision Making
• Desirable teaching/mentorship qualification

Responsabilités

• Lead care planning and support safe admission avoidance
• Provide expert clinical leadership within the multidisciplinary team
• Contribute to staff development, quality improvement, education and service transformation
• Deliver autonomous clinical care in patients' homes with advanced assessment and diagnostics
