Job summary

East Lancashire Hospitals NHS Trust is recruiting an Advanced Clinical Practitioner to join the Intensive Home Support Service (IHSS) in the Burnley area. You will lead advanced practice, provide urgent community assessments, and work across extended hours over a 7-day period, coordinating with ED and NWAS to maximise capacity.

The role requires strong clinical governance, independent prescribing, and collaboration with multidisciplinary teams to deliver high-quality care at patients' homes.

Qualifications

• Advanced clinical practice knowledge for acutely unwell patients.
• Strong communication and advanced assessment skills.
• Experience in governance and risk management with prescribing.

Responsibilities

• Apply advanced clinical practice to assess and manage undifferentiated illness in IHSS patients.
• Provide clinical leadership and supervision across the East Lancashire footprint.
• Collaborate with ED, NWAS and hospital teams to maximise 2-hour UCR capacity.
• Provide oversight and input to IHSS caseload and participate in related projects.
