Job summary

Qualifications

• Must be a registered nurse.
• Community nursing experience preferred.
• Willing to work across a 7-day pattern including weekends and bank holidays.

Responsibilities

• Work in partnership with Primary Care to provide high quality clinical care in the community.
• Support supervision and development of junior staff within the locality health team.
• Collaborate with health and social care partners to manage complex care needs and lead in discharge planning.
