Job summary

Qualifications

• Role requires autonomous professional accountability and effective communication.
• Experience of working within a multidisciplinary Integrated Care Team is desirable.

Responsibilities

• Act with compassion and deliver high-quality care with effective communication.
• Participate in holistic assessment and implementation of care plans for home-based patients.
• Provide safe medicines management following guidelines.
• Develop and maintain relationships with the multidisciplinary team to meet patient needs.
• Promote person-centred practice and challenge non-person-centred care to uphold the culture.
• Keep up to date with clinical developments to uphold care standards.
• Maintain safe and effective clinical skills and record keeping per trust policies.
• Demonstrate competence in clinically relevant procedures.
• Recognise limits in care provision and refer urgent needs appropriately.
• Ensure referrals are managed according to clinical need and assess care holistically.
