Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting for a part-time Registered Nurse or Allied Health Professional to join the Urgent Community Response and Hospital at Home Service. The role focuses on urgent assessments, crisis intervention and discharge support for people with complex health needs, working within a multidisciplinary team across community and hospital settings.

Qualifications

• Registered Nurse or Allied Health Professional required with current registration.
• Part-time role, 30 hours per week.
• Based within Hampshire & Isle of Wight NHS system.
• Focus on urgent community response and hospital at home care.

Responsibilities

• Undertake holistic clinical assessments and develop person-centred care plans.
• Deliver urgent interventions and proactive case management.
• Support patients with frailty and complex health needs to prevent hospital admissions.
• Contribute to timely discharge planning and discharge support.
• Provide leadership, supervision and support to junior staff and students.
• Engage in service improvement, innovation and quality initiatives.
