Job summary

Mid and South Essex NHS Foundation Trust is seeking a friendly and proactive Support Worker to join the Hospital at Home administrative team in Essex. You will coordinate referrals, manage patient records, and support the clinical team with day-to-day administration, helping to deliver hospital-level care at home.

Based at Broomfield Hospital, you will be part of a multidisciplinary team with occasional travel to Basildon & Southend teams. Travel expenses for work journeys are reimbursed.

Qualifications

• Knowledge of Trust IT systems.
• S1 knowledge.
• Microsoft Office good working knowledge.

Responsibilities

• Being the friendly first point of contact for patients, carers, and healthcare professionals.
• Keeping our service organised by managing referrals, patient records, and clinical administration.
• Supporting the coordination of patient visits and ensuring information reaches the right people at the right time.
• Monitoring inboxes, answering calls, and helping to resolve queries efficiently.
• Ordering supplies and helping to keep our teams equipped to deliver outstanding care.
• Helping the Business admin to produce reports and maintaining data that help shape and improve our service.
• Based at Broomfield Hospital, you'll be part of a friendly and supportive multidisciplinary team who are helping patients to receive outstanding care closer to home. From time to time, you'll also provide support to our Basildon & Southend Hospital at Home teams in their office, making access to a car essential. Travel expenses incurred for work-related journeys will be reimbursed.
