Job summary

Via is recruiting a Recovery Practitioner for the Homelessness Specialist role in Merton. This fixed-term, full-time position involves delivering psycho-social support and coordinating care across multiple agencies to help vulnerable individuals rebuild their lives.

You will work directly with clients, partner agencies, and VIA teams to ensure effective interventions, attend multi-disciplinary meetings, and represent VIA in the local community as part of a holistic service delivery model.

Qualifications

• Experience supporting people affected by homelessness and addictions.
• Ability to work within a multi-agency framework to coordinate care.
• Evidence-based psycho-social interventions and case management techniques.

Responsibilities

• Provide evidence-based psycho-social interventions to vulnerable individuals in Merton.
• Attend and contribute to multi-disciplinary meetings and care planning.
• Represent VIA in local communities and engage households supported by Merton Councils Homeless Persons Unit.
• Coordinate with local council, hospitals, outreach services, housing and other services to ensure a multi-agency approach.
