They may refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations. They provide and promote evidence-based care in line with appropriate clinical guidelines and work within clinical protocols, professional guidelines and codes of conduct. They should be familiar with all local, regional and national policies, procedures and guidelines in the pursuit of highest standards of patient care and work within all relevant policies and procedural guidelines. They challenge professional and organisational boundaries, identify areas for skill/knowledge development and apply these to practice to provide continuity and high quality patient health care. They communicate highly sensitive condition related information to patients, relatives and carers. They audit outcome of care against standards and initiate changes as necessary.
