Job summary
Buckinghamshire Healthcare NHS Trust seeks a Community District Nurse for the Amersham Adult Community Healthcare Team. The part-time post (30 hours/week) provides nursing interventions in patients’ homes, with a focus on high-quality, compassionate care and multi-disciplinary collaboration.
You must have a driving licence and access to a car for work. Induction and skill competency development are provided.
Qualifications
- Registered Allied Healthcare Professional / Registered Level 1 is essential.
- Nurse Degree or Diploma in Health / Nursing Studies is essential.
- Evidence of Continuous Professional Development is required.
- At least 1 year experience as a Band 5 in the community is desirable.
- Post-graduate clinical training in relevant areas is advantageous.
Responsibilities
- Provide nursing interventions in patients' home environment.
- Assess physical and psycho-social needs and plan care based on best available evidence.
- Manage a caseload of patients with long-term conditions to prevent admissions.
- Work with MDT and community ward staff to ensure high-quality service delivery.
- Support induction and ongoing skills development for team members.
