Buckinghamshire Healthcare NHS Trust seeks an aspiring District nurse to develop a career in community nursing in the Chalfont's and Gerrards Cross area. The role offers a full skills development and preceptorship programme to support autonomous practice at home.

You will work with a multidisciplinary team to assess care needs, plan, implement and evaluate care while maintaining high standards under Trust policies and the NMC guidelines.

Qualificações

• Registered Nurse with current NMC PIN or awaiting.
• Evidence of ongoing professional development.
• Post-qualification experience in community nursing desirable.
• Mentoring qualification or willingness to work towards it.

Responsabilidades

• Collaborate with multi-disciplinary team to plan and deliver care.
• Assess needs, plan, implement and evaluate care in patients' homes.
• Support safe and high-quality care per Trust policies and NMC guidelines.
• Manage nursing assistants and delegate tasks where appropriate.
• Ensure mandatory training and appraisal requirements are met.
• Note: internal applicants may have secondment with line manager endorsement.
