Qualifications

• Registered Allied Healthcare Professional with current nursing registration.
• Post-registration qualification in community or district nursing helpful.
• Experience in primary care/community settings preferred.

Responsibilities

• Provide nursing interventions in patients' home environment.
• Deliver care plans and case management for high-risk patients.
• Assess physical and psycho-social needs; coordinate MDT input.
• Ensure medicines management and safety in home visits.
• Support complex care and end-of-life care when needed.
• Contribute to caseload management and service development.
