Job summary

Gloucestershire Health and Care NHS Foundation Trust is seeking a proactive Occupational Therapist to join the countywide Home First team in Gloucester. You will assess, plan, deliver and evaluate rehabilitation for adults across acute, community and home settings to support timely discharge and independence.

We expect HCPC registration and a degree in Occupational Therapy or Physiotherapy, with CSP membership to be considered.

Qualifications

• HCPC registration as an Occupational Therapist or Physiotherapist.
• Degree/Diploma in Occupational Therapy or Physiotherapy.
• Membership of a relevant professional body (e.g. CSP).
• Moving and Handling training.

Responsibilities

• As a member of the Home First team, assess, plan, deliver and evaluate rehabilitation and therapy interventions for adults in acute, community and home settings.
• Manage a diverse clinical caseload, undertaking holistic assessments and treatment planning.
• Support discharge planning, admission avoidance and prevent unnecessary readmissions.
• Supervise and support therapy assistants, students and learners where appropriate.
