Epsom and St Helier’s University Hospitals NHS Trust is seeking an experienced Clinician to join the Home First Service in Surrey Downs. You will provide care from initial history taking to evaluation, using safe clinical decision-making and advanced assessment for older people with complex health and social needs.

You will lead and collaborate with the MDT to support discharge and prevent admission, promoting independence and wellbeing.

Competenze

• Current registration with HCPC/NMC.
• Advanced physical assessment skills.
• UK driving licence and access to a car.

Mansioni

• Clinical assessment, treatment planning and autonomous decision‑making for adults referred from community.
• Coordinate care with the Home First MDT to support discharge and prevent admission.
• Lead and model professional practice within the Surrey Downs Health and Care team.
• Develop integrated processes and communications across Home First Service and the wider system.
