Job summary

Frimley Health NHS Foundation Trust in Fleet is seeking Associate Practitioners to join the new Interim Care and Support team to support patients at home.

You will hold a minimum L5 qualification, manage care plans, supervise junior staff, and contribute to service development. A vehicle and business insurance are required; a 7-day rota includes weekends.

The role offers regular supervision, appraisal, and access to training; the Trust emphasises flexible working to meet service needs.

Qualifications

• Foundation degree, NVQ Level 5 or equivalent evidence
• Experience as HCA, Community Support Worker or Therapy Assistant
• Experience undertaking patient assessments
• Flexible to a 7-day roster including weekends

Responsibilities

• Assess patient care needs and write care plans
• Deliver hands-on care and empower independence
• Line manage junior staff and handle HR issues when needed
• Work as part of a 7-day rota 08:00-20:00
