Job summary

Frimley Health NHS Foundation Trust is recruiting Associate Practitioners to join the new Interim Care and Support Team (ICAST) to support patients in their homes before care packages start.

Successful candidates will have a minimum Level 5 qualification and hands-on care experience, with responsibilities including line management and contributing to team development. A vehicle and business insurance are required for this community-based role, which operates seven days a week 08:00-20:00.

Qualifications

• Minimum Level 5 qualification in a relevant subject.
• Experience in line managing junior staff and HR issue handling.
• Willingness to travel and have access to a vehicle with business insurance.
• Able to work a 7-day shift pattern including weekends.

Responsibilities

• Assess patients’ care needs and write care and support plans.
• Empower patients to be as independent as possible and support their networks.
• Deliver hands-on care and ensure high-quality service.
• Cover a 08:00–20:00 seven-day rota with vehicle available for work.
