Frimley Health NHS Foundation Trust is seeking an Associate Practitioner for the Interim Care and Support Team (ICAST). The role is community-based, delivering assessments, care plans, and hands-on care to patients at home on a seven-day rota from 08:00 to 20:00.

You will assess needs, write care and support plans, empower independence, and support team leadership while line managing junior staff. A valid UK driving license and access to a vehicle are required; visa sponsorship is not available.

Qualifikationen

• Foundation degree or NVQ Level 5, or working towards one.
• Experience as HCA, Community Support Worker or Therapy Assistant.
• Experience undertaking patient assessments.
• Experience undertaking assessments and rehabilitation.
• Recent hands-on experience.

Aufgaben

• Assess patients' care needs
• Write care and support plans using a strengths-based approach
• Deliver hands-on care to patients to a high standard
• Empower patients to be as independent as possible, building on their existing support networks
• Support the leadership and development of the ICAST team
• Line manage junior staff, including development, HR issues and quality of service delivery
• Work a rota across the Intermediate Care service from 08:00 to 20:00, seven days a week, including weekends
• Use a vehicle for work purposes
