Job summary

Qualifications

• Foundation degree, NVQ Level 5 or evidence of working towards one of these qualifications, relevant to this role.
• Previous experience as an HCA, Community Support Worker or Therapy Assistant.
• Previous experience undertaking patient assessments.

Responsibilities

• Assess patients’ care needs and write care plans.
• Empower patients to be independent and utilise their support networks.
• Deliver hands-on care to patients and maintain high-quality service delivery.
• Manage a rota covering 08:00–20:00, seven days a week, and ensure vehicle availability for work.
