Frimley Health NHS Foundation Trust in Surrey is seeking a Rehab Support Worker to help patients in their own homes, delivering high-quality personal care and rehab to promote independence.

You will work across a multidisciplinary team, complete Level 2 Apprenticeship in Care, and may progress via a Senior Healthcare Support Worker Apprenticeship. A driving licence and car access are required.

Qualifications

• Essential criteria: GCSE maths and English (or equivalent).
• Level 2 Apprenticeship in Care or 1 year+ health and social care experience.
• Desirable: Level 3 Apprenticeship in Care and Care Certificate.

Responsabilités

• Provide high quality clinical care to patients in their own homes under indirect supervision.
• Undertake rehab with patients to improve independence at home.
• Promote patient safety and dignity; maintain safe environment.
• Support end of life care as required and coordinate with multidisciplinary team.
