Job summary

East Sussex Healthcare NHS Trust is recruiting a Band 5 Registered Nurse or Paramedic to join the Urgent Community Response (UCR) and Virtual Ward (VW) Service. You will provide direct clinical assessments, treatments and monitoring in patients' homes, aiming to avoid hospital admissions.

You will work in a multidisciplinary team, delivering acute-level hospital-at-home care, with opportunities to develop skills in urgent care, frailty, and seven-day service delivery.

Qualifications

• Valid full UK driving licence and access to a car.
• Nurse or Paramedic registration (Band 5).

Responsibilities

• Deliver nursing or paramedic interventions including clinical observations, medication administration, wound care, intravenous therapies (where competent), and other agreed treatments within scope of practice.
• Develop and evaluate individualised care plans in partnership with patients, carers and the wider multidisciplinary team.
• Support patients to maintain independence and achieve the best possible outcomes while receiving care at home.
• Work collaboratively with medical colleagues, therapists, primary care teams, acute services, social care and voluntary sector partners to coordinate seamless patient care.
• Maintain accurate and contemporaneous electronic clinical records and contribute to effective communication across services.
• Participate in service development, quality improvement initiatives and audit activity.
• Contribute to the delivery of a seven-day service, working flexibly across pathways as required.
