Job summary
East Sussex Healthcare NHS Trust is seeking an experienced Band 6 Urgent Community Response Practitioner (Nurse/Paramedic) to deliver rapid assessments and interventions in the community. You will work within a multidisciplinary team to ensure timely care, help prevent hospital admissions, and support patients in their own homes.
The role requires current registration as a Nurse (Adult) or HCPC-registered Paramedic, with a full UK driving license and access to a car.
Qualifications
- Current registered nurse (Adult) or HCPC-registered paramedic.
- Experience in urgent/community settings is desirable.
- Ability to work across multidisciplinary teams to deliver community care.
Responsibilities
- Provide rapid assessment and intervention for patients in the community.
- Collaborate with a multidisciplinary team to create individualized care plans.
- Stabilize patients and arrange treatments, referrals, or hospital admissions as needed.
- Work with GPs, social care and community services to maintain independence at home.
- Lead and support junior staff and provide supervision as required.
- Contribute to development and implementation of clinical protocols.
