Job summary

East Sussex Healthcare NHS Trust is seeking an enthusiastic Band 5 Nurse or Paramedic for the Urgent Community Response and Virtual Ward service in Eastbourne. The role provides patient‑facing, in-home assessments and treatment, enabling rapid care at home and reducing unnecessary hospital admissions.

You will work within a multidisciplinary team to deliver safe, high-quality care, including clinical monitoring, referrals, and flexible seven-day service delivery across pathways.

Qualifications

• Registered Nurse with active NMC registration.
• Diploma or degree in nursing relevant to role.
• Postgraduate qualification relevant to urgent community care.
• Full UK driving licence and car for work.

Responsibilities

• Deliver high-quality clinical care in patients' homes as part of the UCR and VW pathways.
• Undertake timely assessments for patients referred to the 2-hour Urgent Community Response service.
• Provide ongoing face-to-face monitoring and management of patients in the Virtual Ward.
• Recognise, assess and respond to clinical deterioration and escalate as needed.
• Deliver nursing or paramedic interventions including observations, medications and wound care.
• Collaborate with medical colleagues, therapists, primary care teams and social care to coordinate seamless care.
