Job summary
East Sussex Healthcare NHS Trust in Eastbourne seeks a Band 6 Urgent Community Response Practitioner to deliver rapid assessments and interventions in the community. You will work within a multidisciplinary team to keep patients safe at home, coordinate timely treatments, and support hospital avoidance where appropriate.
Applicants should be an experienced Registered Nurse (Adult) or HCPC registered Paramedic, with a full UK driving license and access to a car.
Qualifications
- Registered Nurse (Adult) with UK registration or HCPC Registered Paramedic.
- Full UK driving license and access to a car.
- Experience in urgent community care or hospital-at-home settings preferred.
Responsibilities
- Provide rapid assessment, intervention and care to patients in the community.
- Collaborate with a multidisciplinary team to create individualized care plans.
- Stabilize patients and arrange necessary treatments, referrals or admissions as needed.
- Work with GPs, social care and community services to maintain independence at home.
- Lead and support junior staff with guidance and supervision.
- Contribute to development and implementation of clinical protocols.
