Job summary

East Sussex Healthcare NHS Trust is recruiting an experienced Band 6 Urgent Community Response (UCR) Practitioner to deliver rapid assessments and interventions in the community, supporting the Virtual Ward service and Hospital@Home initiatives. Applicants must hold UK registrations and be ready to work across East Sussex.

The role requires a full UK driving licence and car access with rotating shifts across 8:00–22:00, including weekends.

Qualifications

• NMC registration and HCPC registration as applicable.
• Paramedic registration with HCPC or equivalent.
• Teaching and Assessing qualification or equivalent.
• Evidence of continuing professional development to diploma level.
• Community Nursing or Nurse Practitioner qualification.

Responsibilities

• Provide rapid assessment and care to patients in the community.
• Collaborate with a multi-disciplinary team to create individualized care plans.
• Stabilise patients and arrange treatments, referrals or admissions as needed.
• Work with GPs, social care and community services to maintain patient independence at home.
• Lead and supervise junior staff and contribute to clinical governance.
• Develop and implement clinical protocols to maintain high care standards.
