Job summary

Essex Partnership University NHS Foundation Trust is seeking a Rehabilitation Assistant (HCA) to support the senior team in delivering patient-centered rehabilitation at home when needed and in clinical settings. You will work within a multidisciplinary team to assist with assessments, treatments, and daily living activities, responding to emergencies with appropriate action.

Qualifications

• Experience in patient rehabilitation or care within a multidisciplinary team.
• Commitment to patient dignity, respect and privacy.
• Ability to work flexibly and follow infection control policies.

Responsibilities

• Assist patients with daily living activities and rehabilitation goals.
• Observe and report changes in health and environment to senior staff.
• Assist in planning and delivering care with therapists, including in-patient beds when required.
• Contribute to care planning and risk management, following relevant policies.
• Collect specimens and monitor dietary needs and fluid intake as required.
