ESSEX PARTNERSHIP UNIVERSITY NHS FOUNDATION TRUST seeks a dedicated home-care support worker to assist registered nurses in delivering palliative care to patients in their own homes. Qualifications

Phlebotomy experience or training.
Ability to perform continence assessments.
Order and manage necessary equipment.
Accurately measure and record vital signs.
Maintain accurate patient records and documentation.
Excellent communication with patients, families, and colleagues.

Responsibilities

Visit stable palliative care patients at their homes.
Take blood, measure blood pressure, pulse and other vitals.
Document patient care in electronic records.
Coordinate with registered nurses to plan daily visits.
