Job summary

Epsom and St Helier University Hospitals NHS Trust is seeking a Rehabilitation Assistant to join the Care Home Support Team. The role provides proactive, person-centred rehabilitation and support to residents in care homes, particularly older people living with frailty and complex health needs.

Working under supervision of registered clinicians, you will deliver delegated rehabilitation programmes, support continence care, and help embed rehabilitation into everyday care.

Responsibilities

• Deliver and progress delegated rehabilitation programmes aimed at maximising independence, mobility, function and quality of life.
• Support residents with mobility, transfers, daily living activities and continence.
• Undertake delegated functional assessments and observations and escalate concerns to supervising clinicians.
• Contribute to care planning, MDT discussions and discharge planning.
• Coach care home staff to promote mobility and rehabilitation in everyday care.
