Essex Partnership University NHS Foundation Trust is seeking a Community palliative care healthcare assistant to support registered nurses in home visits across Canvey Island. You will visit stable patients, take vitals, assist with phlebotomy, continence assessments, and ensure accurate record keeping in the electronic system.

Formación

• Strong communication and teamwork abilities.
• Experience with measuring vitals and basic nursing procedures is desirable.
• Ability to document care accurately in electronic records.

Responsabilidades

• Visit stable palliative patients in their homes.
• Take blood, measure blood pressure, pulse, and other vitals.
• Order equipment and support continence assessments as needed.
• Document patient care accurately in the electronic record and communicate with the nursing team.
