Job summary

Unique Community Services, part of the Catalyst Care Group, is seeking Healthcare Support Workers to deliver comprehensive mental health care at home for people with complex needs. You will lead person-centered services, ensuring compassionate, effective care aligned with our community values.

Responsibilities include mobility support, vital signs monitoring, meal assistance, companionship, medication administration, and accurate record-keeping. Driving license is required for community work.

Qualifications

• Minimum one year of experience in healthcare settings, community or home-based care.
• Experience managing complex health needs, including mental health, autism, learning disabilities, and diverse populations.
• Valid driving license required for community work and transporting service users.
• PMVA training is required.
• Experience supporting people with complex health needs.

Responsibilities

• Provide mobility support and basic moving and handling.
• Monitor vital signs (blood pressure, temperature, pulse).
• Assist with meals, nutrition, hydration, and maintaining a clean environment.
• Offer companionship, emotional support, and, when needed, financial assistance.
• Administer medication and maintain accurate documentation.
