Job summary

Unique Community Services, part of the Catalyst Care Group, is recruiting Healthcare Support Workers to deliver high-complexity mental health care at home in West Yorkshire. You will lead person-centered services for individuals with challenging behaviours and personality disorders, ensuring care is compassionate and effective in the community.

The role involves flexible shifts (4–8 hours or longer) and varies with each care package.

Qualifications

One year of previous experience in healthcare settings, in community or home-based care.
Experience in managing complex health needs, with a focus on mental health, autism, learning disabilities and working with diverse populations.
Driving license is mandatory for community work and transport of the people we support.
PMVA training is required.
Experience supporting people with complex health needs.

Responsibilities

Provide mobility support, including basic moving and handling.
Monitor vital signs such as blood pressure, temperature, and pulse.
Help with meal preparation, nutrition, hydration, and maintaining a clean environment.
Offer companionship, emotional support, and, when needed, financial assistance.
Administer medication and maintain accurate documentation and records.
