Unique Community Services, part of the Catalyst Care Group, delivers high-complex mental health care at home for people with challenging behaviours and personality disorders. We are hiring Healthcare Support Workers to lead person-centered services directly in clients’ homes.

Qualifications

• At least 1.5 years of healthcare experience in community or home-based care.
• Experience managing complex health needs, with mental health, autism, learning disabilities experience.
• Driving license is mandatory for community work and transport of those supported.
• PMVA training is required.
• Experience supporting people with complex health needs.

Responsibilities

• Provide mobility support, including basic moving and handling.
• Monitor vital signs such as blood pressure, temperature, and pulse.
• Help with meal preparation, nutrition, hydration, and maintaining a clean environment.
• Offer companionship and emotional support, and financial assistance when needed.
• Administer medication and maintain accurate documentation and records.
