Job summary

Unique Community Services monitors and supports individuals with complex mental health needs in their own homes. The Healthcare Support Worker role focuses on delivering compassionate, person-centered care, managing challenging behaviours, and aligning with the company values of compassion, impact and community.

Shifts vary and may include longer hours based on care packages. Applicants should have at least 1.5 years of healthcare experience in community or home care, driving license, PMVA

Qualifications

• 1.5+ years of healthcare experience in community or home care.
• Experience managing complex health needs, including mental health.
• Driving license is mandatory for community work.
• PMVA training is required.
• Experience supporting people with complex health needs.

Responsibilities

• Provide mobility support, including basic moving and handling.
• Monitor vital signs such as blood pressure, temperature and pulse.
• Assist with meals, nutrition, hydration and maintaining a clean environment.
• Offer companionship and emotional support; assist with financial needs when required.
• Administer medications and maintain accurate documentation and records.
