Job summary

Dorset HealthCare is recruiting a Community Rehabilitation Assistant to join the Christchurch and Bournemouth Intermediate Care team. You will deliver rehabilitation-focused care to patients in their own homes and support independence under supervision.

The role supports preventing hospital admissions and facilitating early discharge as part of the Dorset Frailty Hospital at Home service, with a flexible schedule including 8am-8pm coverage across seven days a week.

Qualifications

• Deliver rehabilitation-focused care to individuals in their own homes.
• Assist patients to regain independence through re-education and task adaptation.
• Training to carry out venepuncture and ECGs.

Responsibilities

• Deliver rehabilitation care in patients' homes.
• Support early discharge and prevent hospital admissions.
• Collaborate with multidisciplinary team in community settings.
