Job summary

Stour Surgery in Christchurch is seeking a compassionate Healthcare Assistant with a special interest in frailty and dementia to join the SCOT team. You will support housebound patients to remain safe, well and independent at home, delivering planned and proactive care.

The role involves health assessments, frailty screening, blood sampling, and collaboration with GPs and community teams to improve outcomes for vulnerable patients.

Qualifications

• Experience as a Healthcare Assistant in care settings.
• Interest in frailty and dementia (training can be provided).
• Able to work confidently in patients' homes.
• Strong communication and clinical assessment skills.
• Understanding safeguarding and person-centred care.
• Ability to work in a multidisciplinary team.

Responsibilities

• Undertaking annual reviews and health assessments for patients with frail and long-term conditions.
• Completing frailty assessments and supporting personalised care planning.
• Taking and recording clinical observations.
• Undertaking blood tests for housebound patients.
• Supporting dementia and frailty care, including working with patients, families and carers.
• Identifying and escalating safeguarding concerns.
• Identifying changes in patients’ health, wellbeing or social circumstances and escalating concerns appropriately.
• Supporting care coordination and liaising with GPs, Nurses, community teams and other professionals.
• Providing health promotion and preventative care.
• Working collaboratively with the wider practice team to improve outcomes for our most vulnerable patients.
