Dorset HealthCare is recruiting a Community Rehabilitation Assistant to join the Christchurch and Bournemouth Intermediate Care team. You will deliver rehabilitation-focused care to patients in their own homes and support independence under supervision.

The role supports preventing hospital admissions and facilitating early discharge as part of the Dorset Frailty Hospital at Home service, with a flexible schedule including 8am-8pm coverage across seven days a week.

Are you looking for an opportunity to develop your skills and would like to work in an Intermediate Care Team? We have an exciting opportunity for for someone who is keen to be part of a team which is at the forefront of developments in the NHS., We are looking for a Community Rehabilitation Assistant to join the Christchurch and Bournemouth Intermediate Care team (CBIC). We are a team of Occupational Therapists, Nurses, Physiotherapists, Assistant Practitioners and Community Rehabilitation Assistants. We are able to offer flexible working options including compressed working days within our 8am-8pm service over 7 days per week.

The team works to prevent avoidable hospital admissions, enable earlier discharges from hospital and provide acute rehabilitation for a wide variety of conditions. The team is integral to the Dorset Healthcare Frailty Hospital at Home service and the successful candidate will be working within this service regularly as part of their remit. The team's caseload comprises of a wide variety of medical problems including orthopaedic, cardiac, neurological, respiratory and multi pathology.

The successful candidate will be expected to facilitate independence of patients' functional abilities by implementing re-education and adaptation of tasks under the guidance of qualified staff. The successful candidate would also be expected to undertake training to be able to carry out clinical skills such as venepuncture and ECGs.
