Job summary

Dorset HealthCare University NHS Foundation Trust is seeking a Community Rehabilitation Assistant to join the Christchurch and Bournemouth Intermediate Care team. The role involves delivering rehabilitation in patients' homes within the CBIC remit and working flexibly across an eight-to-eight service seven days a week.

You will work with a multidisciplinary team to prevent hospital admissions and support earlier discharges, undertaking training for venepuncture and ECGs and completing the Care

Qualifications

• Completion of the Care Certificate is necessary.
• Be eligible to work in the UK without visa sponsorship.

Responsibilities

• Deliver high-standard rehabilitation-focused care to individuals in their own homes.
• Facilitate independence by re-educating and adapting tasks under supervision.
• Undertake training to perform clinical skills such as venepuncture and ECGs.
