Job summary

Essex Partnership University NHS Foundation Trust seeks a Care Assistant to support the senior team in providing care and physical rehabilitation for patients needing short-term recovery at home after an acute event. The role aims to help patients achieve maximum independence and live as safely as possible.

You will work with therapists to deliver direct care and rehabilitation, encourage activities of daily living, monitor health, and contribute to care planning and risk management.

Responsibilities

• Work as part of the multidisciplinary team and, when necessary, on your own initiative.
• Provide direct care and rehabilitation to the patients served by the team.
• Protect and maintain patients' dignity and respect, observing religious and cultural practices.
• Encourage patients to participate in activities of daily living and engage in therapeutic activities.
• Assist patients to work towards rehabilitation goals to enhance independence and wellbeing.
• Participate in manual handling of patients per the Trust’s Manual Handling Policy.
• Follow Infection Control Policy and wear appropriate PPE.
• Observe, record and report changes in patients' health and environment to senior staff.
• Be familiar with steps in medical emergencies and act accordingly.
• Complete observations including urinalysis and blood sugar monitoring as required.
• Support the senior team during prelim, first, and review visits to patients.
• Collaborate with therapists to support effective assessment and treatment, including in-patient beds if needed.
• Assist in planning work with the senior team and contribute to care planning and risk management.
