Job summary

West Suffolk NHS Foundation Trust invites applications for a Band 5 Nurse in the Urgent Community Response team, delivering high-quality care in patients' homes across West Suffolk. You will triage, assess, plan and provide unplanned care, including end of life and palliative care, with support for preceptorship for newly qualified nurses.

Qualifications

• RN qualification (Part 1 of register) or equivalent.
• Current NMC registration.

Responsibilities

• Triage, assess, plan, implement and evaluate all aspects of patient care, mainly unplanned to include palliative and end of life care.
• Drive to patients' homes and deliver an individualised, evidence-based plan of care.
• Liaise with GPs, Social Services and other health professionals to ensure needs are met.
• Deliver IV therapy and participate in MDT meetings when needed.
