Job summary

City Health Care Partnership CIC is seeking a Community Nurse to support people in their own homes across Willerby and surrounding areas. You will deliver holistic nursing care, assess needs, and develop person-centred care plans in collaboration with patients, families and the multidisciplinary team.

Joining our district nursing team, you’ll travel between patient visits, maintain accurate records and contribute to high-quality, safe care.

Qualifications

• Registered Nurse qualification with current professional registration.
• Understanding of community nursing and evidence-based practice.
• Experience working in a healthcare environment and as part of a team.
• Basic IT skills and a commitment to learning and professional development.
• Ability to travel between patient visits and work flexibly to meet service needs.
• Strong communication skills, including discussing sensitive information.

Responsibilities

• Carry out holistic assessments and develop person-centred care plans
• Deliver nursing care and treatment safely within your scope of practice
• Work with multidisciplinary colleagues to coordinate and improve patient care
• Maintain accurate patient records and report clinical concerns appropriately
• Support patients and carers with information, guidance and health education
