Job summary

City Health Care Partnership CIC is recruiting a Community Nurse to assess patients and deliver safe, personalised nursing care in patients’ homes. You’ll work within the District Nursing Service to support people’s independence and choice.

You’ll collaborate with a care team, travel between visits, maintain accurate records and contribute to learning and development of junior colleagues and students.

Qualifications

• Registered nurse with current professional registration.
• Healthcare experience including supporting patients, families and carers.
• Knowledge of patient assessment, evidence-based practice and safe care.
• Clear communication, teamwork, organisation and basic digital skills.
• Flexibility to travel between patient visits.

Responsibilities

• Complete holistic assessments and develop, deliver and review personalised care plans.
• Provide safe clinical care, including treatments appropriate to your skills and professional practice.
• Recognise changes in patients’ needs and share relevant information with colleagues and partner services.
• Maintain accurate, confidential records and report clinical incidents appropriately.
• Support the learning and development of junior colleagues and students.
