Job summary

Newcastle Hospitals NHS Foundation Trust is seeking a Community Cardiac Rehabilitation worker (Band 3) to support home-based exercise programmes for patients post-MI, PCI, cardiac surgery and heart failure. You will visit patients in their homes, assist with prescribed programmes, and work alongside a Band 7 physiotherapist.

The role requires car ownership with business insurance, and clinical supervision will be provided.

Qualifications

• NVQ Level 3 in appropriate subject (or on pathway) or equivalent qualification / experience
• Good literacy and numeracy skills
• Level 3 Exercise to Music qualification
• BACPR Completion of a Trust or other HCA development programme

Responsibilities

• Assist Registered Nurses or Physiotherapists in providing high quality patient care within the community setting and in patients' homes
• Check baseline observations and assist with prescribed exercise plans
• Cover cardiac rehabilitation centre group classes in the absence of the fitness instructor
• Work with a physiotherapist and nursing staff to deliver home and community exercise programmes
