Forward Trust is recruiting a Health and Wellbeing Practitioner to support adult men and young men in the Criminal Justice System. You will deliver personalised wellbeing services, including 1:1 and group sessions, assess needs, and develop care plans to promote positive changes and resilient wellbeing.

The role involves collaborating with colleagues, subcontractors and partner agencies, managing a caseload, and supporting individuals during prison-to-community transitions. DBS checks apply.

Qualifikationen

• Ability to manage an allocated caseload and provide personalised support.
• Experience in assessing needs and developing care plans.
• Strong interpersonal skills for engaging with service users and partner agencies.
