Job summary

The Forward Trust is recruiting a Health and Wellbeing Practitioner for HMP Brixton in Brixton. This full-time role delivers drug and alcohol recovery support to men in custody, helping address substance misuse and wellbeing to enable positive futures beyond prison.

You will work with healthcare professionals and partner agencies, complete assessments, develop care plans, run group sessions, and ensure accurate records while maintaining safeguarding and confidentiality.

Qualifications

Responsibilities

• Manage a caseload, assessments and individual care plans.
• Deliver one-to-one interventions, group sessions and structured programmes.
• Provide harm reduction advice for drug and alcohol misuse.
• Coordinate care with healthcare professionals and partner agencies.
• Maintain records and support service targets and quality standards.
• Identify safeguarding concerns and ensure service user safety.
