Forward Trust's Connections Team at HMP Millsike is recruiting a More Than My Past Practitioner for a full-time role (35 hours per week). You will deliver wellbeing support to people in custody, run group workshops and mentor service users to achieve lasting change, while collaborating with prison staff and external partners.

• Passionate about health, wellbeing and recovery.
• Skilled at building positive relationships and motivating others.
• Confident delivering both one-to-one and group-based interventions.
• Organised, proactive and able to manage a varied caseload.
• Comfortable working within a multidisciplinary environment.
• Committed to safeguarding, inclusion and person-centred support.

Responsabilidades

• Delivering 1:1 support, group interventions and wellbeing workshops
• Coaching and mentoring service users to achieve positive, lasting change
• Supporting and developing prisoner Health & Wellbeing Champions
• Promoting healthy lifestyles, recovery and wellbeing across the prison
• Organising health promotion events and awareness campaigns
• Building strong partnerships with healthcare, mental health and substance misuse services
• Supporting the delivery of a varied programme of wellbeing activities, including evenings and weekends
• Maintaining accurate records, care plans and case management documentation
