Job summary

The Forward Trust in Hertfordshire is seeking a Health & Wellbeing Practitioner to support individuals in the criminal justice system. The role focuses on harm reduction, mental health support and recovery planning within an integrated healthcare framework.

You will manage caseloads, conduct assessments, develop SMART care plans and deliver individual and group interventions to promote wellbeing and successful resettlement. A compassionate, client-centred approach is essential.

Qualifications

• Experience in substance misuse and/or mental health services.
• Ability to build trusting, professional relationships with clients.
• Ability to deliver structured 1:1 and group psychosocial interventions.
• Experience completing risk assessments and designing SMART care plans.

Responsibilities

• Provide harm reduction advice, guidance and interventions.
• Complete comprehensive assessments, risk assessments and SMART care plans.
• Deliver structured 1:1 and therapeutic group-work sessions on substance misuse and mental health.
• Use motivational interviewing techniques to support behaviour change.
• Support clients to develop recovery goals and life skills for release and resettlement.
• Collaborate with internal and external services to ensure continuity of care.
