Job summary

North Staffordshire Combined Healthcare NHS Trust is seeking a dedicated worker to support people with learning disabilities in managing health needs. You will work under supervision to contribute to care planning and deliver person-centered support.

The role involves participating in multidisciplinary discussions, adapting communication to complex needs, and applying NICE guidelines to interventions.

Qualifications

• NVQ Level 2/3 in Direct Care or equivalent qualification.
• Good literacy, numeracy and basic IT skills (GCSE A-C or equivalent).
• Experience in a learning disability setting relevant to the role.

Responsibilities

• Empower people to realise and maintain their potential in relation to health and well-being.
• Contribute to and create plans for people with learning disabilities to manage identified health needs.
• Deliver interventions in line with NICE guidelines and local procedures.
• Participate in multidisciplinary case discussions.
• Develop accessible, person-centred support plans with service users and carers.
