Job summary

Hft is seeking a dedicated Support Worker to empower learning disabled adults to live life their way, with a focus on independence and dignity. Your daily role includes enabling choices, supporting participation in activities, and promoting wellbeing within a warm, inclusive team.

You’ll contribute to safe, person-centred care, attend health appointments, and help individuals achieve their goals. A Health and Social Care qualification and a compassionate attitude are valued.

Qualifications

• Patience and respect for people’s choices and independence.
• Strong communication to build trusting relationships.
• Willingness to learn and work towards a Health and Social Care qualification.
• Flexibility to cover evenings, weekends, and sleep-in shifts.

Responsibilities

• Empower people to make choices and live independently.
• Encourage participation in social, leisure, work and community activities.
• Promote health, safety, wellbeing and safeguarding.
• Support attendance at health appointments and medication management.
• Build independence and self-belief through step-by-step support.
• Assist with daily life and personal care when needed.
• Maintain accurate records and safeguarding practices.
