CNTW NHS Foundation Trust seeks a Band 3 Clinical Support Assistant to join the Sunderland Learning Disability Community Treatment Team. You will support assessments, interventions, and reviews under registered practitioners, delivering person-centred care with a multidisciplinary team.

You will maintain electronic health records and contribute to care planning, risk assessment, and safeguarding. A compassionate, adaptable, and team-focused approach is essential for this role.

Competenze

• Completion of a competency-based programme within probationary period or equivalent demonstrable experience.
• Knowledge of care related procedures, clinical observation, relevant legislation.
• Level 3 qualification in a healthcare related subject or equivalent demonstrable experience (desirable)

Mansioni

• Assist in delivering care to service users with learning disabilities or autism under a registered practitioner.
• Implement and evaluate care within prescribed framework to promote recovery and wellbeing.
• Maintain electronic health records of treatments completed.
• Work as part of the team; provide in-reach into hospital environments as required.
• Provide compassionate, empathetic, respectful, and dignified care.
• Engage with supervision and appraisal as per Trust policy.
