Job summary

Practice Plus Group is recruiting a Health Navigator for HMP Winchester on an 18-month contract to support Early Days in Custody within our primary care team. You will bridge HMPPS and healthcare providers to improve joint care and identify risk factors in early custody.

We offer a bespoke induction, competency framework, and regional/national career development. Applicants must have the UK right to work and pass security checks.

Qualifications

• Qualified RGN with experience in primary care.
• Ability to handle confidential/sensitive issues.
• Understanding of change management and new ways of working.
• Pro-active, engaging, confident in running forums and sharing information.

Responsibilities

• Bridge HMPPS and healthcare providers to enable improvements in joint care.
• Improve identification of risk factors during early days in custody.
• Offer a visible presence in the First Night Centre / Induction Wing.
• Address vulnerabilities in early days in custody including reducing self-harm.
• Create greater awareness of healthcare and social care options.
• Support access to services and assist with applications for people with diverse needs.
• Improve partnership working between services including social care and Safer Custody teams.
• Improve engagement rates into healthcare and reduce DNA appointments.
• Assist with patient choice regarding healthcare interventions.
• Promote Health Champions and social prescribing and manage patient forums post ACCT.
• Input into complex case meetings.

About the role

We have an exciting opportunity for a Health Navigator – an 18‑month contract to join our primary care team with a focus on Early Days in Custody, at HMP Winchester, a category B Prison located in central Winchester. HMP Winchester is a Category B Local remand prison located in Central Winchester accommodating approximately 670 male prisoners. There is onsite parking for all staff and it is a short walk from Winchester train station.

Hours

37.5 hours a week working Monday‑Friday, 9am‑5pm. No weekends.

Salary

This position is full time, you’ll receive an annual salary of £33,000 per annum - depending on experience.

What you’ll be doing

• Bridge HMPPS and healthcare providers to enable improvements in joint care.
• Improve identification of risk factors during early days in custody
• Offer a visible presence in the First Night Centre / Induction Wing
• Address vulnerabilities associated with early days in custody inclusive of reducing self-harm
• Create greater awareness of healthcare and social care options and the range of services available
• Support access to those services i.e., use of PODS, application process especially for people from diverse backgrounds or with specific needs
• Improve partnership working between services – particularly social care, Safer Custody teams
• Improve engagement rates into healthcare and reduce did not attend appointments
• Assist with improving patient choice regarding healthcare interventions.
• Further support social prescribing and promote and manage Health Champions.
• Set up and run specific patient forums particularly relating to post ACCT
• Input into complex case meetings
